What is PACE?
Neighborhood PACE (Program of All-Inclusive Care for the Elderly) is an alternative to nursing home care that provides customized healthcare and social support to help aging adults remain independent and living in their homes.
Neighborhood PACE participants receive tailored medical care from the finest professionals who specialize in aging and chronic conditions. Our medical team works with therapists, dieticians, social workers, and home care nurses to create a customized care plan for each Neighborhood PACE participant to ensure optimal health, safety and well-being.
Participants receive all health care including primary and specialist services (other than emergency services) from Neighborhood PACE or a contracted provider authorized by Neighborhood PACE. Participants may be fully and personally liable for the costs of unauthorized, out- of-network care or services.
Who is PACE for?
Neighborhood Healthcare PACE Serves adults 55 and older with medical conditions who:
- Need special care and assistance to remain living safely in their home
- Meet the Skilled Nursing Level of Care requirements as determined by the California Department of Health Care Services
- Can live in the community without jeopardizing their safety
- Live in our Riverside and San Bernardino County service areas:

How do I schedule an evaluation?
How much will it cost?
If a participant qualifies for Medi-Cal, there is no monthly premium for PACE programs. Seniors that are not Medi-Cal eligible, but have Medicare, pay a monthly premium, but do not have deductibles or copayments. Private pay options are available for participants who do not have Medi-Cal or Medicare.

Is transportation provided?
Neighborhood PACE picks up PACE participants from their homes and safely transports them to and from the Neighborhood PACE Center and doctor’s offices and back to their homes.
What do I do at the PACE center?
Medical care, therapy, social support, and opportunities for socialization and recreation are offered at our PACE center. Our day center offers services to participants in one convenient location. Transportation is provided to all participants.
Primary medical care clinic visits, dental care, therapy, occupational therapy, social work, and more are provided at Neighborhood PACE.
What if I can’t come to the center? Do I have to go to the PACE center?
Attendance at the PACE center is optional, although participants who participate in the wellness and social aspects of PACE see the best outcomes.
Can I disenroll from PACE?
Yes, you can disenroll from pace at anytime. Please speak with a Neighborhood PACE staff member for more information.
